Quality of life in Chinese Parkinson's disease patients with Subthalamic Deep Brain Stimulation in Hong Kong: A single center prospective study

摘要Objective: Investigate the outcome of bilateral subthalamic nucleus (STN) Deep Brain stimulation (DBS) in Chinese Parkinson's' disease patients in Prince of Wales Hospital, Hong Kong
Background: Bilateral subthalamic nucleus (STN) Deep Brain Stimulation (DBS) has been shown to improve motor function and activities of daily living (ADL) in Parkinson's disease (PD). Apart from these, quality of life (QOL) of PD patients should also improve. We would investigate the effect of STN DBS on QOL among Chinese PD patients residing in Hong Kong
Methods: During the period 2009‐2017, we evaluated the QOL of 44 PD patients who underwent bilateral STN DBS in Prince of Wales Hospital by using Unified Parkinson's disease rating scale (UPDRS) and PD Questionnaire (PDQ‐39) at baseline (before surgery), and at 1year, 2 years, and 3 years after surgery
Results: 44 PD patients were recruited with 14 female and 30 male. The mean age was 56.4 years old. The mean disease duration was 10.7years. The UPDRS part3 (medication OFF condition) at baseline was 41.6, while the post DBS surgery UPDRS part 3 (medication OFF with DBS ON condition) 1year, 2 years and 3 years were 17.4, 20.9 and 21.4 respectively, and that showed significant improvement in mobility. Moreover, for PDQ‐39 sub‐ score in mobility item, the scoring were 58.5, 37.3, 45.6, and 43.5 at baseline, post DBS 1 year, 2 years and 3 years respectively. In PDQ‐39 sub‐ score in ADL also showed improvement as scoring were 42.8, 26.4, 25.9 and 29.9 at baseline, post DBS surgery 1 year, 2 years and 3 years respectively. Both sub‐scores showed statistical significant improvement (p value <0.05).
Conclusions: Parallel to the improvement in UDPRS part 3, the global QOL score also improved significantly and maintained over 3years after surgery among Chinese PD patients. However, our result showed that the improvement in emotional, social support, cognitive and communication were only mild and insignificant. Extra efforts should be made to attend these specific needs of the patients.
