A New Reproducible and Clinically Relevant Metaphyseal Fracture Model in Osteoporotic Rats to Study Enhancement of Bone Healing

摘要Introduction: Most osteoporotic fractures occur at the metaphyseal bone region but few models exist and the healing is poorly understood. There are 2 purposes to the current study 1) to conduct a systematic review to analyze current osteoporotic metaphyseal fracture animal models 2) to create a new osteoporotic metaphyseal fracture model Materials and Methods: Literature search was performed on the Pubmed, Embase and Web of Science databases and relevant articles were reviewed. For the creation of the model, 26 6-month old Sprague-Dawley rats were obtained. All rats underwent induction of osteoporosis. A 6-hole T-shaped mini-plate and screws were used to fix the plate to the distal femur and an osteotomy was performed. X-ray of the femur was taken weekly and the rats were euthanized at 1, 2, 4 and 6 weeks post-surgery. For analysis, sections cut longitudinally were stained with Hematoxylin-Eosin. Results: A total of 19 studies were included in the systematic review. We propose that an animal model should satisfy the following criteria: 1) induction of osteoporosis, 2) unilateral complete osteotomy or defect at the metaphysis and 3) internal fixation. There was complete bony bridging in all animals at 6 weeks. Intramembranous ossification was observed during the healing phase. Discussion and conclusion: Upon detailed analysis of the current models we have derived a recommendation and created a valid and reproducible metaphyseal fracture model that satisfies these criteria, which can provide a platform for future quality studies to enhance osteoporotic fracture healing.
