Evaluating Safety and Efficacy Of Combination Medication Of Aspirin And Oral Panax Notoginseng Preparation On Cerebral Infarction: A Systematic Review And Meta-Analysis

摘要Background and Aims:
Stroke is the second leading cause of death and a major cause of disability worldwide. The combination medication of aspirin and oral Panax notoginseng preparation on cerebral infarction is commonly used in China. Panax notoginseng can significantly inhibit platelet aggregation and plasma coagulation. However, it is unclear whether Panax notoginseng increases risk for serious bleeding events and death in cerebral infarction patients with when used togetherthe use of with aspirin. Therefore, we sought to evaluate the clinical safety and efficacy of aspirin-PNP combination through a systematic review.
The aim of the study was to To detect possible adverse effects and efficacy of combined use of aspirin-oral Panax notoginseng preparation combination on cerebral infarction.
Nine electronic English and Chinese databases (PubMed, EMBASE, AMED, IPA, CENTRAL, CNKI, VIP, CBM and Wanfang) were searched up to February 2018. The risk of bias tool as described in the Cochrane Handbook was used to evaluate the risk of bias of the included studies. Randomized controlled trials (RCTs) and quasi-RCTs comparing aspirin with aspirin-oral Panax notoginseng preparation (a crude herb or an extract) combination were eligible for inclusion.
Sixteen RCTs were included. Risk of bias was assessed as high in two studies, and unclear in the remaining thirteen studies. Ten of the included trials reported adverse events. There was no obvious difference between aspirin-oral Panax notoginseng preparation (PNP) combination group and aspirin group in reported adverse events. The pooled results showed that aspirin-PNP combination was more effective in reducing mortality, blood coagulation and cerebral infarction recurrence rate than aspirin alone. The combination also better improved the quality of life and neurological function in cerebral infarction patients.
The data from RCTs suggested that aspirin-PNP combination was likely safe. This review indicated some beneficial effects about aspirin-PNP combination. However, more rigorously designed trials are needed to confirm these findings, we need more rigorously designed trials.
