Comparison of PRF and hydrogel-delivered PRP on healing of partially torn ACL in a rat model

摘要Introduction & Aims: After anterior cruciate ligament (ACL) partial tear, tissue regeneration can be enhanced with bioactive factors. Platelet-rich plasma (PRP) is a safe source, but localization is crucial. We aim to compare the treatment effects of PRP delivered by a thermogel (mPEG-PLGA) with platelet-rich fibrin (PRF) on healing of ACL partial tear.

Method: A standard-sized lesion (0.3mm) was created in ACL near femoral footprint in 100 rats using 30G needle. PRP and PRF were prepared from other rats on day of surgery by centrifugation according to previous reports. Rats were randomly assigned to 4 groups (n=24 per group): untreated, hydrogel only, PRP-loaded hydrogel, and PRF. At 2 and 6 weeks post-operation, knee samples were collected for tensile load-to-failure test of femur-ACL-tibia complex (n=8) and histology (n=4). Eight additional un-traumatized rat knees were included in mechanical test for determination of mechanical strength of intact ACL. Statistical analysis was performed by 2-way ANOVA at α=0.05.

Results: Tensile strength of the untreated ACL partial tear decreased over time (p=0.017), but changes in stiffness was not significant (p=0.154). No difference was observed among four groups at 2 weeks post-operation. At 6 weeks post-operation, PRP-hydrogel group showed a better restoration of ultimate load (p=0.042) and stiffness (p=0.002) as compared to hydrogel-only group, while the effect of PRF was not significant (p=0.218). Increased cellularity was observed at the lesion site in PRP-hydrogel and PRF groups.

Conclusions: PRP delivered by thermosensitive hydrogel can promote healing of ACL partial tear without surgical repair, while PRF was less effective. It may open up the potential of non-operative treatment for ACL partial tear.
