Development of an online multidisciplinary self-management programme for community-dwelling stroke survivors and caregivers

摘要Introduction: Challenges for stroke survivors to self-manage their health following hospital discharge are enormous and self-management programmes currently centre on face-to-face sessions. Survivors and their caregivers often face difficulties in participating in these programmes due to transportation problems or limited sessions. The lack of timely support may compromise their quality of life and increase survivors’ readmission risk.
Aim: We aimed at developing an online programme to support survivors’ self-management and enhance caregivers’ outcomes in a timely and flexible manner.
Methods: A video online platform and a call centre will be set up to facilitate the interactions between the participants and healthcare providers. An expert panel from various healthcare disciplines, including nursing, medicine (Western and Chinese) and pharmacy cooperates to address the survivors’ and caregivers’ needs over a distance. The user-centered design methodologies were adopted to engage potential users in the design of the programme. Eight discovery interviews were conducted to examine the survivors’ experience and stories after stroke, their perceived information and care needs, habits in self-management, survivors’ and caregivers’ expectations about the online service, and aspects of delivery and receipt process that may bring about satisfaction with the service. The key
objective was to gain an in-depth understanding of the needs and perceptions of survivors and their caregivers, and to include user input in the development of a tailored online programme.
Discussion: and conclusion This is the first online multidisciplinary self-management programme for Chinese population and we expect this approach of care would be a new trend in future.
