The differentiation of viral and non-viral wheeze in the hospitalized children by the evaluation of immune signatures in the nasopharyngeal aspirate

摘要Respiratory infection is the most common reason for hospitalization in children and most of the respiratory tract infections are of viral origin. Viral wheeze is very common in infant and young children and most of them outgrow the problem, while some would have persistent wheeze with different intensities. Aeroallergens and environmental stresses would also trigger wheezing.

In this prospective study, we obtained 200 nasopharyngeal aspirate (NPA) samples from subjects aged 0 – 8 years old with respiratory symptoms who were admitted to the Prince of Wales Hospital. The supernatant of the NPA would be used for protein level (CXCL11 and CXCL10) measurement, while the cell pellet would be spun down and the gene expression (CXCL10, IFIT2, OASL) would be quantified using qPCR. These parameters would serve as the host signatures to identify patients with viral, non-viral or no infection, based on the reported threshold. At the same time, the microbial detection result from the hospital microbiology laboratory would be used to compare with the result generated from the host signatures, in terms of sensitivity, specificity and evaluate based on ROC curve.

This project would validate if these host signatures could serve equally well to predict the nature of infection in the local population. The result would assist the differentiation of viral and non-viral wheeze in a particular episode with laboratory evidence and assist in the patient care.
