Estimating the Geometric Effects on the Rupture Risk of Intracranial Atherosclerotic Plaques

AbstractBackground & Purpose: The atherosclerotic plaques rupture could cause ischemic and hemorragic stokes. The local stress condition could reflect the risk of plaque rupture. To investigate if the geometry of a plaque could reflect its rupture risk, we calculated the local stress by the computational fluid dynamics (CFD) simulation.

Methods: We built a tube model with the length of 75mm and the radius of 1.5mm. A stenosis was located 25mm from the inlet. By varying the lumen radius from 0.75mm, 0.636mm to 0.5mm at the throat of stenosis, we got stenoses with 75%, 82% and 89% severity in area. We derived three eccentricities by locating the stenosis center at the center line of artery, and moving it 0.375mm and 0.75mm away towards the vessel wall. We adjusted the length of the proximal and distal segments of the stenosis: 2mm-5mm, 3mm-5mm, 4mm-5mm, 5mm-5mm, 5mm-4mm, 5mm-3mm, 5mm-2mm. Totally, we have 3*3*7=63 geometrically different models. We calculated the maximum and averaged values of wall shear stress (WSS) and axial plaque stress (APS) on the proximal and distal segments of the stenosis. We estimated the effects of the severity (S) , the eccentricity (E), lengths of proximal and distal segments (P and D) on WSS and APS (P-value<0.05 as significant).

Results: The significant factors were: S and P for maximum WSS on proximal and distal segment; S,Eand P for maximum and averaged APS on the proximal segment,and averaged WSS on the proximal segment; S,E and D for the maximum APS on the distal segment; S, E, P and D for averaged WSS and APS on the posterior segment.

Conclusion: The severity is significant for all the mechanical parameters. The eccentricity and proximal/distal lengths also have effects on the APS and WSS. The geometric characteristics reflected the risk of plaque rupture.
