Residual symptoms of idiopathic REM sleep behavior disorder with standardized medication treatment: prevalence and clinical correlates

摘要Objectives: The current study aimed to determine the prevalence and correlated factors of residual symptoms in a large scale of patients with iRBD who had received sufficient dose of clonazepam and/or melatonin.
Methods: A total of 167 out of 209 eligible patients with iRBD responded to follow-up (79.9%). Residual symptoms were defined as having sleep-related injury or potentially injurious behaviors to self and/or bed partner at least once per year after receiving two first-line medication treatment. Baseline demographic characteristics, clinical assessments, and polysomnographic data were compared.
Results: There were 25 patients (15.0%) having residual symptoms after receiving two medications. Residual symptoms group was more likely to be male (92.0% vs 76.8%, p = 0.084), younger (61.9 ± 5.8 years vs. 68.2 ± 7.6 years, p < 0 .001), having earlier onset of RBD symptoms (57.3 ± 6.1 years vs 62.6 ± 8.7 years, p = 0.005), and smoking (28.0% vs 10.6%, p = 0.017) when compared with response group. Patients with residual symptoms also had severer RBD symptoms as measured by RBDQ-HK (50.8 ± 14.7 VS. 40.0 ± 17.3, p = 0.004), higher doses of clonazepam (1.3 ± 0.6 mg vs. 0.9 ± 0.5 mg, p = 0.001) and melatonin (7.7 ± 3.4 mg vs 1.6 ± 2.9 mg, p < 0.001), and shorter REM sleep latency (84.8 ± 39.5 minutes vs. 124.0 ± 78.0 minutes, p < 0.001) than their counterparts. No differences (such as AHI, REM sleep duration, and EMG activity level during REM sleep) were found between groups.
Conclusion: Residual symptoms are common in patients with iRBD even after receiving two first-line medications. Those patients with residual symptoms of RBD have different demographic and clinical characteristics with those responded patients. The findings in the current study have implications for clinical management of RBD.
