Activated Osteocyte Dendrite Proliferation In Enhanced Osteoporotic Metaphyseal Fracture Healing In Response To Low-Magnitude High-Frequency Vibration

摘要Introduction: Osteoporotic fracture has a high prevalence among the elderly with impaired healing properties and were mostly concentrated in metaphyseal region. Osteocytes are responsible for mechano-sensing and initiate the repair mechanism during the healing cascade. Low-magnitude high-frequency vibration (LMHFV) is a non-invasive biophysical intervention reported to promote osteoporotic fracture healing.
Objective: We hypothesized that mechanical loading altered the Osteocyte Lacuno-canalicular Network (LCN) in the ovariectomized metaphyseal fracture model.
Methods: For osteoporosis development, 72 bilateral ovariectomies were performed. A metaphyseal fracture was created and fixed. Rats were randomized to either LMHFV or control groups. Weekly radiographs were taken at week 1, 2, 6 post-fracture and MicroCT was used to evaluate the change of bone properties at fracture sites. Undecalcified histology was used to examine the LCN system both qualitatively and quantitatively by Scanning Electron and Confocal-Imaris (*p<0.05).
Results: Serial radiographies and micro CT showed an improvement of healing after the treatment of LMHFV. Morphological assessments of osteocytes revealed that the LCN exhibited a more vibrant morphology after vibration with a 11% increase in dendritic branch points, 26% increase in dendritic length at week 2 (p=0.02) and an 50% increase in length at week 6.
Conclusion: Our findings revealed that vibration promoted the rate of healing in the osteoporotic metaphyseal model, and this is the first report to indicate that there was a better healing outcome with re-vitalized morphological changes in the osteocytes. The dramatic alterations suggested that osteocyte could respond to mechanical stimulation in the form of LMHFV and might play a role in accelerating osteoporotic fracture healing.
