Associations between insomnia and hypoglycemia in Hong Kong Chinese patients with type 2 diabetes

Insomnia impacts negatively on insulin resistance and glucose metabolism in type 2 diabetes (T2D). However, its relationship with hypoglycemia is under-explored. Here, we aimed to examine the association between insomnia and hypoglycemia in T2D.
Between July 2010 and June 2015, T2D from Hong Kong Diabetes Registry[1] were invited to complete validated questionnaires to assess their sleep habits when they attended comprehensive assessments for treatment goals and diabetes related complications. Insomnia was defined as Insomnia Severity Index score>14[2]. Insomnia subtypes, namely difficulty initiating sleep (DIS), difficulty maintaining sleep (DMS), early morning awakening (EMA) and nonrestorative sleep (NRS), were defined as the occurrence of self-reported symptoms >thrice weekly in the past 1 month. Hypoglycemia was defined as any experience of hypoglycemia >once monthly in the past 3 months.
3940 T2D were recruited in this present analysis [mean age 54.0±8.7 (SD) years, 58.8% men]. 9.9% had hypoglycemia and 9.7% had insomnia. Patients with T2D and insomnia were more likely to have hypoglycemia than those without insomnia (16.7% vs 9.2%, p<0.001). After adjustment for age, gender, body mass index and disease duration of diabetes, insomnia remained significantly associated with hypoglycemia [Odds Ratio (OR)=1.88, p<0.001]. Similarly, insomnia subtypes were independently associated with hypoglycemia (OR for DIS, DMS, EMA and NRS were 1.68, 1.67, 1.65 and 2.07, all p<0.05).
Insomnia and hypoglycemia are prevalent in patients with T2D. T2D with insomnia have increased the risk of hypoglycemia compared to their counterparts without insomnia and deserve more clinical attention to avoid hypoglycemia.
