How to Improve Our Palliative Care Services? Voices of Different Stakeholders

AbstractBackground and Objectives: Optimising the care at the end of life has been prioritized in the policy agenda globally over the recent years due to high prevalence of progressive debilitating diseases and ageing population. This study seeks to explore different perspectives from a range of stakeholders on the development of palliative and end-of-life care in Hong Kong.

Methods: Informants were recruited based on a quota sampling framework. Individual interviews were conducted respectively with a range of health care providers and people involved in the services from different care sectors, carers, patients, whereas focus group interviews were conducted with frontline care staff in community care settings. Data collection started since January 2017, with data analysis being conducted iteratively, and will continue until data saturation. Thematic analysis was conducted to identify themes emerged from the data.

Results: As of February 2017, 22 individual interviews and five focus group interviews were conducted with five doctors, six nurses, five social workers, three administrators, two paramedics, two lawyers, two researchers, one journalist, six family carers and four volunteers. The preliminary findings showed that the major challenges in the existing palliative care services are the interface between curative and palliative medicine, the continuity of care between hospital and community care, and the transfer of knowledge between health professionals and lay public.

Conclusion: Three major service gaps were identified from the findings that hinder the development of palliative care services. These underscore the importance of strategies to encourage earlier collaboration between acute care and palliative care team, to enhance inter-sectoral communication that facilitate seamless transitions, and to improve public awareness towards palliative care.
