Pulse wave velocity is significantly higher in psoriatic arthritis patients with severe stenosis in coronary artery

摘要Background: Arterial stiffness correlates with coronary atherosclerosis in the general population. However, it is unclear whether there is similar correlation in patients with psoriatic arthritis (PsA).

Methods: 64 PsA patients (38 males; age: 50±11 years) without overt cardiovascular disease were included. Coronary atherosclerosis was evaluated by coronary computed tomography angiography (CTA). Arterial stiffness was determined as brachial-ankle pulse wave velocity (PWV) and augmentation index (AIx).

Results: 38 (59.4%) patients had at least 1 coronary plaque, while 31(48.4%) had non-calcified plaque (NCP). Mild-moderate (<50%) and severe (≥50%) coronary arterial stenosis was found in 11 (17.2%) and 5 (7.8%) patients, respectively. The patients with coronary plaques were significantly older (52±10 vs. 47±12 years, p=0.043), while patients with severe stenosis had a longer PsA disease duration (21±11 vs. 10±9 years, p=0.012). There was no significant difference in PWV (1463±229 vs. 1534±228 cm/s, p=0.233) and AIx (24.4±9.1 vs. 23.3±6.5 %, p=0.596) between patients with and without coronary plaques (Figure 1A). The results were similar while comparing between patients with and without NCP (Figure 1B). However, the patients had stenosis in coronary arterial had higher PWV (no stenosis: 1468±230; mild-moderate: 1574±197; severe: 1721±147 cm/s, p=0.033; Figure 1C). After adjusting for age, gender and PsA disease duration, patients with severe stenosis still had higher PWV compared with other
patients (p=0.047).

Conclusion: Although there was no significant correlation between arterial stiffness and the presence of plaque or NCP in PsA patients, PWV was significantly higher in patients with severe stenosis.
