Efficacy, safety and mechanism of commonly used Chinese herbal medicine formulae for treatment of endometriosis

To compare therapeutic and side effects and anti‐endometriosis mechanisms of commonly‐used Chinese herbal medicine formulae SFZY, XFZY, GXZY, WJD, FZHY, THCQ and THSW in an experimental endometriosis model in mice.
Endometriosis was induced by syngeneic endometrium intraperitoneal transplantation. After 1 week, the mice were randomly administered with either Chinese herbal medicine formula, Dienogest as positive control or vehicle as negative controls for 3 weeks. All Chinese herbal medicine formulae were authenticated and prepared under standard decoction and concentration methods.
Materials and methods:
Lesion size, weight and histology were compared. Adhesion, invasion, proliferation, angiogenesis, survival and pain nerve innervations in the lesions were examined by quantitative PCR for mRNA expression and/or immunohistochemistry/ELISA for protein expression. Body, uterus and ovary weights, female reproductive hormone levels, endometrial glands and thickness, and follicle counts were monitored.
Amongst all formulae, only SFZY, THCQ and THSW significantly decreased the lesion size and weight by 42.3%, 48.3% and 48.5%, respectively. Under SFZY, THCQ and THSW treatment, invasion Plau and proliferation Pcna mRNA expression were significantly decreased. Adhesion Itgv3, apoptosis Mapk1 and Nfb, and angiogenesis HIF1 and VEGFA mRNA expression have no significant different from negative control. However apoptosis caspase‐3 mRNA expression was significantly decreased. There were no significant changes in peritoneal fluid PGE2 levels and PGP9.5 nerve innervations; body, uterus and ovary weights; blood E2, P, FSH and LH levels; endometrial gland counts and thickness; and antral follicle counts. However, Dienogest significantly decreased the lesion size by 38.9%, in additions adhesion Itgv3 and apoptosis Mapk1 mRNA expression were also significantly decreased and increased, respectively.
Only 3 commonly‐used Chinese herbal medicine formulae SFZY, THCQ and THSW have significant anti‐endometriosis effects with no major reproductive side‐effects. However the effects is not superior to Dienogest in terms of decreased lesion growth and development and inhibited anti‐endometriosis mechanisms. The active components for the therapeutic effects is still unknown.
