Abnormal ultrastructure of the Osteocyte-lacuno-canalicular system in Adolescent Idiopathic Scoliosis - A new novel finding

AbstractObjective
Adolescent Idiopathic Scoliosis (AIS) is a complex three-dimensional spinal deformity of unknown etiology. Recently, our group reported that abnormal endocortical modeling and deranged trabecular bone structure in AIS with high-resolution peripheral quantitative computed tomography (HR-pQCT). Osteocytes, the most abundant bone cells, are critical players in the regulation of endocortical bone formation. This study aimed to analyze the ultrastructure of the
osteocyte-lacuno-canalicular network (collectively as OLC system) from bone biopsies in AIS and age-matched controls.

Methods
In this case-control study, we recruited 5 girls with severe AIS and 5 age-matched controls for intraoperative iliac crest bone biopsies under strict IRB approved protocol. Fresh bone biopsies were embedded in methylmethacrylate (MMA) and examined by Goldner’s trichrome method, acid-etched scanning electron microscopy (SEM) and confocal microscopy with quantitative fluorescein isothiocyanate (FITC)-Imaris technique.

Results
Compared with the aged-matched controls, bone histomorphometry showed the lower osteocyte density and higher osteoid volume in trabecular bone of AIS patients. In controls trabecular bone, SEM showed the well-organized osteocytes lacunae with spindle-like shape and the high interconnectivity of osteocytes with a large number of long canaliculi protruding from the lacunae. While AIS showed osteocyte lacunae were disorganized and rounder shape with a decrease in connectivity by fewer, shorter and cluttered canaliculi. These qualitative ultrastructural changes
were confirmed with FITC-Imaris technique. Quantitative analysis of FITC-Imaris technique showed differences with a mean of 38% shorter, 44% less canaliculi and 28% larger lacunar volume in AIS OLC system.

Conclusion
To the best of our knowledge, for the first time, qualitative acid etched SEM and quantitative FITC-Imaris technique were used to illustrate the presence of clusters of abnormal ultrastructure of the OLC system in AIS iliac crest biopsies in comparison with age-matched controls. Further studies will help to advance our understanding of the association between the abnormal bone quality and ultrastructure and its possible contributions to the etiopathogenesis of AIS.
