Gender Differences in Age-related Changes in Bone Density and Microarchitecture in Chinese

摘要Introduction: We hypothesise that gender difference in fracture risk has its origin in differences of age-related changes in bone density and microarchitecture. This study aimed to describe gender differences in age-related changes in bone density, and cortical and trabecular microarchitecture in a large cohort of Chinese males and females.

Methods: A total of 611 Chinese (193 male and 418 female) subjects aged from 10 to 80 years were recruited from local community. Volumetric bone mineral density (vBMD) and microarchitecture were assessed using high-resolution
peripheral quantitative computed tomography (HR-pQCT) at the distal radius and tibia.

Results: There were significant between-gender differences in young subjects (20-39 years). Males had larger bone, higher trabecular vBMD, thicker trabeculae, and thicker cortices, but not necessarily a higher cortical vBMD. Significant between gender differences became more prominent with advancing age, with males having a better preservation of bone quality than females. Decrease in vBMD in males was mainly related to thinning of trabeculae, whereas in females was related to losing of trabeculae, which is more detrimental to bone strength. Decrease of cortical bone quality was also more prominent in females who appeared to have a more rapid decrease in cortical thickness after midlife than males.

Conclusion: Young males achieved better bone quality than females and during ageing, males had a better preservation of bone quality, both of which contribute to the lower fracture risk in older males.
