Persistent osteopenia in adolescent idiopathic scoliosis (AIS) - Factors predisposing to generalized osteopenia, a cross-sectional and longitudinal investigation

AbstractGeneralized osteopenia in adolescent idiopathic scoliosis is well documented. No large-scale cross-sectional or longitudinal study has been conducted to determine factors contributing to low bone-mineral-density (BMD) and whether low BMD is persistent throughout adolescence. We aimed to determine the link between BMD, physical growth, calcium intake and bone turnover in 594 AIS girls aged 11-16 years and compared to those of 300 age-matched healthy girls. We also followed up a group of AIS girls aged 12-17 years to track the changes of BMD during peripuberty. Results showed that a significantly faster rate of anthropometric growth and higher bone-turnover in AIS might lead to increased bone length. Calcium intake in AIS was too low (< 400 mg/day) to be sufficient for optimal bone mineralization. Osteopenia in AIS may therefore result from sub-optimal bone-mineralization qualitatively and quantitatively, and thus fails to catch-up with abnormally escalated bone growth during peripubertal period. Results of longitudinal data showed that BMD accretion in AIS was persistently reduced throughout adolescence when compared with age-matched controls. AIS with more severe curve-severity appeared to have lower BMD. Promotion of a higher bone-mass in AIS may be important to strengthen the bone strength of the scoliotic spine which may modify the natural course of scoliosis progression and to reduce the risk of osteoporosis in future. © 2006.
