Amniotic fluid gamma-glutamyl-transferase for the prediction of biliary atresia in fetuses with non-visualized gallbladder using a validated analytic platform and a local reference chart: a retrospective study

摘要ntroduction: Amniotic fluid gamma-glutamyl-transferase (AFGGT) level may help to identify biliary atresia in fetuses with non-visualized gallbladder (NVFGB). The objectives of the study were to validate the utility of a serum/plasma-matrix-based GGT assay on amniotic fluid samples, to establish a local gestational-age-specific AFGGT reference range, and to evaluate the efficacy of AFGGT for the prediction of fetal biliary atresia in pregnancies with NVFGB using the reference chart constructed.
Methods: The analytic performance of GGT assay on amniotic fluid samples using a serum/plasma-matrix-based GGT assay on the Roche cobas c502 analyser was first evaluated. AFGGT levels in confirmed euploid singleton pregnancies from 16+0 to 22+6 gestational weeks were determined using the same analyser to establish a local gestational-age-specific reference range from 2.5th to 97.5th centile. Using this local reference chart, the positive predictive value (PPV) and negative predictive value (NPV) of AFGGT <2.5th centile for the identification of fetal biliary atresia in euploid pregnancies with NVFGB were determined.
Results: The serum/plasma-matrix-based GGT assay was able to determine GGT levels in amniotic fluid samples reliably and accurately. Using the local gestational-age-specific AFGGT reference range constructed, the NPV and PPV of AFGGT <2.5th centile for the prediction of fetal biliary atresia in pregnancies with NVFGB were 100% and 25% (95% CI [0, 53]) respectively.
Conclusion: In pregnancies with NVFGB, AFGGT ≥2.5th centile probably excludes fetal biliary atresia. AFGGT <2.5th centile is not diagnostic of fetal biliary atresia, but fetuses with AFGGT below this level should be referred for early postnatal investigation.
