AbstractEighty-five percent of Hong Kong citizens have not heard of advance care planning (ACP) and advance directives (AD). An empowerment project, (HOPE), proposed in this chapter, consists of three key areas: (H) Health care professionals' empowerment, (O) Older people/family members' empowerment, and (PE) Public empowerment. Four multi-dimensional and interactive phases will be arranged to promote the awareness of the Hong Kong community about their right and autonomy of ACP and AD; to empower nurses and medical social workers (MSWs) with competence and confidence to provide one-on-one consultations on ACP and AD to older people and their families; and to engage older people (residential care home elders) and their family members in the discussion of end-of-life care and ultimately, to develop their living wills and AD documents. © 2012 Nova Science Publishers, Inc.
