AbstractPurpose : The objective was to elucidate the herb- drug interactions between water extracts of GR and carbamazepine (CBZ) in a model of rats for the purpose of safety ahead of clinical investigations.
Results : CBZ was metabolised into Carbamazepine- 10,11- epoxide (CBZE) that reached a maximum concentration in 10 hours. The AUC (0-t) CBZE in the acute and sub- acute phase of CBZ treatment were 184.3 mg/L*h and 153.8 mg/ L*h respectively, indicating significant self- induction (16.5%, p = 0.037) that reduces the AUC (0-t) due to CYP450 enzyme activity. When combined with GR, the reduction in AUC (0-t) value between the acute and sub- acute phase of treatment was no longer observed, indicating that GR may interact with CBZ and reduce the self- induction of CBZ. The T 1/2z value (17.44 hour) and V z/F value (10.46 L/kg) of CBZE in sub- acute CBZ treatment were significantly higher than T 1/2z value (11.45 hour, p = 0.003) and V z/F value (4.22 L/kg, p = 0.0002) in acute CBZ treatment, respectively. GR also significantly reduced the V z/F value in the sub- acute phase of treatment with CBZ (p = 0.002).
Conclusion : We found that GR interacted with CBZ in reducing the effect of enzyme induction via CYP450. This may increase the therapeutic level of CBZ in the sub- acute phase of treatment at 2 weeks.
