Mechanistic study of a Chinese herbal paste on relieving osteoarthritic knee pain

AbstractOsteoarthritis (OA) is a common musculoskeletal disease. Pharmaceutical interventions are effective to alleviate OA pain but their potential adverse effects restrict their long-term use. Certain Traditional Chinese Medicine (TCM) formulae are generally agreed to be effective against OA and are non-toxic. However, scientific researches to demonstrate their effectiveness and mechanisms of action are not enough to make them being recognized worldwide. This study aims to provide an evidence-based scientific support on the efficacy of a topical Chinese herbal paste, named DAEP, including Dipsaci Radix, Achyranthis Bidentatae Radix, Eucommiae Cortex and Psoraleae Fructus, on the relief of OA pain. An OA rat model has been developed by anterior cruciate ligament transection (ACLT) on right hind leg followed by forced running on treadmill. DAEP was applied topically on the injured knee join for eight weeks. The OA pain was accessed via Incapacitance test and CatWalk gait analysis. Related gene and protein expressions on the articular cartilage were analyzed via qPCR and Western blot, respectively. Results showed that DAEP treatment exhibited a pain relieving effect on OA by maintaining the static weight ratio of the affected leg throughout the study. Analysis on gait parameters also revealed that DAEP improved that walking quality of the OA rat. DAEP significantly suppressed the upregulation of gene expression of interleukin (IL)-6, tumor necrosis factor (TNF)-α, and inducible nitric oxide synthase (iNOS) of the articular cartilage. It exhibited its effect via the nuclear factor (NF)-κB pathway by suppressing the phosphorylation of IκB kinase αβ (p-IKKαβ) and cyclooxygenase-2 (COX-2) protein expression. To conclude, DAEP was effective to ameliorate OA pain by suppressing the expression of inflammation cytokines via the NF-κB pathway.
