In vivo efficacy of a Chinese herbal paste on the treatment of osteoarthritis

摘要Osteoarthritis (OA) is a common musculoskeletal disease but the effectiveness of
pharmacological interventions remains controversial. Although total knee replacement is an
effective intervention, it is expensive and invasive. Certain Traditional Chinese Medicine
(TCM) formulae are generally agreed to be effective against OA and are non-toxic. However,
there is inconsistent in literature regarding the benefits of TCM due to the diverse and poorly
documented treatments. This study aims to provide an evident-based scientific support on the
efficacy of a topical Chinese herbal paste, named DEAP, including Dipsaci Radix,
Eucommiae Cortex, Achyranthis Bidentatae Radix and Psoraleae Fructus, on the treatment of
OA.
A rat OA model has been developed by receiving anterior cruciate ligament transection
(ACLT) on right hind leg followed by forced running on treadmill. DEAP was applied
topically on the injured knee join for eight weeks. Results showed that DEAP exhibited a
pain relieving effect on OA by increasing the bearing load of the affected leg by 20%
significantly after 8 weeks of treatment. Analysis on gait parameters also revealed that DEAP
improved that walking quality of the OA rat. Toluidine blue staining demonstrated that the
articular cartilage of the animals received ACLT and forced running was damaged severely
compared with the Sham control. However, DEAP could not prevent the articular cartilage
against deterioration. Concentration of the inflammation markers in serum of the rats was too
low to be detected, indicating that there was no systemic inflammation in the animal
throughout the experiment. DEAP was found effective to ameliorate pain and improve
walking quality of the animal suffering from OA.
